Pilomatricoma of the external auditory canal

Pilomatricoma was first described in 1880 by Malherbe as a calcifying epithelioma, but this later changed to benign tumour arising from the hair follicle matrix cells. It typically presents firm, painless well-circumscribed mass children. They are most commonly found head and neck, particularly peri-auricular area. slow growing asymptomatic, generally appearing solitary multiple lesions can be seen. While there is paucity of case reports on pilomatricoma auricle, even fewer reported cases external auditory canal pilomatricomas. We report 15-year-old male who initially presented with an enlarging lesion, which caused conductive hearing loss. An excision biopsy performed under general anaesthetic. Histology returned consistent pilomatricoma.

We present a case of bilateral otitis externa that did not respond to local treatment. Cutaneous biopsies revealed bilateral amyloid depositions secondary to multiple myeloma. Persistent, identical bilateral canal lesions may be the only manifestation of treatable systemic disease and should be biopsied, even though their bilaterality argues against malignancy.
